Varix of Iris (Colour Photography Anterior Segment, Ultrasound Biomicroscopy)

Iris and Ciliary Body -> Tumors/Neoplasmas -> Benign
Patient: 88 years of age, female, BCVA 0.4 at OD, 0.5 at OS; IOP 16/15 mmHg. Ocular Medical History: increased IOP at OD under local antiglaucomatous therapy. General Medical History: arterial hypertension, diabetes. Main Complaint: increased IOP at OD. Purpose: to present prominent round dark brown iris mass as varix node of the iris. Methods: Photography Anterior Segment, Ultrasound Biomicroscopy. Findings: Colour Photography Anterior Segment: at OD peripherial circumscribed prominent round dark brown iris mass with smooth surface. Ultrasound Biomicroscopy: prominent and liquid-filled lesion within the peripherial iris stroma. Discussion: Diagnosis of an iris varix was made clinically. It was shown (1) that iris varix might show a complete regression during follow-up of months. Possible etiological factors are congenital or acquired changes of the iris vessels and alterations of the iris vessel walls in pseudoexfoliation syndrome. Although varices of the iris are rare, they should be included in the differential diagnosis of prominent iris lesions. Matlach et al. (2) suggested, that symptomatic iris varices can be treated safely by combined argon and diode laser photocoagulation, facilitating a less invasive approach than surgical excision.
